Why Some Patients Don’t Improve — Even When Your Technique Is Correct

Many experienced therapists—massage therapists, and increasingly physical therapists and chiropractors—eventually encounter a frustrating reality:

You apply the right techniques.
You follow sound clinical reasoning.
And yet… patients do not improve.

These are often the most challenging cases:

  • persistent neck and upper back tension
  • chronic headaches and fatigue
  • stress-related disorders
  • post-viral conditions such as Long COVID

In these situations, the limitation may not be your knowledge or your technique.
It may be something deeper.

By myself a few years ago, treating painful back and limb disorders and sleep disorders that usually, after numerous treatments, showed good evidence of improvement—I could not achieve the same results. I am talking about cases that, during decades, were not a challenge for me to resolve. And suddenly, everything changed.

Below is the story—please spend a few minutes to read.

During the last four years, I and most clinicians have started seeing clients suffering from combined symptoms: tension headaches, cognitive decline, TMJ dysfunction, brain fog, sleep disorders, and dizziness.

Little by little, I came to the conclusion that the panic during the COVID pandemic—when the media constantly reported scenarios resembling the end of the world, and when schools and businesses were locked down and real social isolation occurred—placed the entire population under tremendous chronic stress for months and even years.

How I Came to the Conclusion That We Are Dealing With Chronic Stress-Related Disorders

My conclusion comes from years of experience working with Olympic teams.

These athletes were exposed to:

  • constant emotional pressure
  • extreme physical workload

Imagine a 23-year-old athlete with:

  • a resting pulse of 45 beats per minute
  • training three hours, twice a day
  • not allowed to stop until the heart rate reached 220 beats per minute

The physical and emotional stress load was extraordinary.

If the assigned sports massage practitioner failed to provide quality post-event sports massage, some athletes developed what we informally called “burnout.”

They demonstrated:

  • emotional exhaustion
  • cognitive symptoms
  • sleep disturbances
  • autonomic irregularity (the most important biomarker)

When I began observing the massive rise of chronic stress-related disorders in the general population after COVID, I immediately recognized the similarity.

Beyond Technique: The Missing Element in Clinical Practice

Over decades of clinical work, one principle becomes increasingly clear:

Technique alone does not determine outcomes.

What matters is:

  • a deep understanding of what you feel under your hands
  • how you interpret that information
  • how you adjust your approach in real time

This is the trained sense of touch.

It is what allows experienced clinicians to:

  • detect subtle tissue changes
  • recognize patterns not visible on the surface
  • adapt treatment moment by moment

And it is rarely taught in a structured way.

Understanding Complex Conditions from a Broader Perspective

Conditions such as Long COVID and chronic stress-related disorders are increasingly understood as involving multiple interacting systems.

These may include:

  • neuroinflammatory processes
  • autonomic nervous system dysregulation
  • vascular and fluid dynamic changes
  • persistent myofascial tension patterns

Clinically, these patients often present with consistent findings:

  • cervical and suboccipital tension
  • thoracic restriction
  • reduced diaphragmatic movement
  • brain fog, sleep disturbances, memory difficulties
  • anxiety, depressive symptoms
  • TMJ dysfunction and chronic headaches

There is exact relationships between these conditions,the broader perspective helps explain why purely local or technique-based approaches may not be sufficient.
